Understanding Keloid Treatment Factors
Every keloid is unique and requires a customised approach. Several factors influence the treatment plan:
- Location: Keloids in areas like the chest and cheeks are more aggressive and have a higher chance of recurrence, requiring more intensive protocols. Ear and abdominal keloids are less likely to regrow.
- Number of Keloids: Patients with multiple keloids benefit from a combination of oral medication and non-surgical therapies.
- Size: Small keloids often respond well to non-surgical options, whereas larger keloids may require cryotherapy or surgery.
- Stage: Early-stage keloids (within the first few months) are more responsive to treatment and less likely to recur. Older keloids are generally easier to treat.
- Family History: Patients with relatives who have keloids have a higher risk of recurrence.
- Gender: Male patients statistically have a higher recurrence rate.
- Previous Treatments: Post-surgical recurrences often require more aggressive treatment protocols.
- Lifestyle and Budget: Patient’s ability to attend regular sessions and financial considerations also impact the treatment plan.
Treatment Options for Keloid & Hypertrophic Scars
Our non-surgical methods aim to reduce keloids with minimal downtime:
1. Corticosteroid Injections
Triamcinolone acetonide (40 mg/ml) is injected into the keloid to reduce inflammation, shrink scar tissue, and inhibit collagen formation.
- Frequency: Injections are administered every 3 weeks until the keloid is flat.
- Procedure:
- Local anaesthetic is applied to minimise discomfort.
- Steroid is injected into the scar tissue.
- The treated area is covered with plasters for one hour.
- Maintenance: Patients may need occasional top-ups if keloid regrowth occurs.
2. Chemotherapy Injections (5-FU)
Fluorouracil (5-FU) is injected locally to stop keloid cell division and destroy scar tissue. It is most effective when combined with corticosteroids.
3. Verapamil Injections
Reduces collagen production and inflammation, often used in combination with steroids for enhanced results.
4. Botulinum Toxin
Injected into and around the scar to reduce its size, alleviate tension, and minimise symptoms like itching.
5. Hydrocolloid Patch Therapy
Non-invasive occlusion therapy to soften scars, reduce pain, and improve skin texture.
6. Pressure Therapy
Specialised pressure earrings for ear keloids reduce blood supply and help prevent regrowth.
Surgical excision is recommended for larger or treatment-resistant keloids. Options include:
- Excision & Stitches: Removing the keloid with precise suturing.
- Cryotherapy: Freezing the keloid with liquid nitrogen.
- Shaving: Gradual removal of scar layers.
Post-Surgical Care:
- A course of steroid or 5-FU injections is often required to prevent recurrence.
- Silicone sheets or compression therapy aid in healing and flattening scars.
Our advanced triple therapy, developed by Dr. Salinda Johnson, provides a highly effective solution for ear keloids.
Step 1: Keloid Removal
- Pre-Treatment: Avoid blood-thinning medication, alcohol, and supplements 2 days prior. Eat normally before the procedure.
- Procedure: Local anaesthetic numbs the area before keloid removal via shaving or excision. Minimal bleeding occurs, thanks to electro-surgical technology. Steroids are injected immediately after removal to prevent regrowth.
- Post-Treatment:
- A wound care kit and antibiotics will be provided.
- Follow-up arranged after 4 weeks.
- An emergency contact number is given.
Step 2: Preventative Measures
- Steroid Injections: A minimum of 5 sessions every 2–4 weeks.
- Silicone Sheeting: Applied daily for 12–24 hours to soften scars and prevent recurrence.
